ЭФФЕКТИВНОСТЬ КРИОЛИПОЛИЗА И ЕГО КОМБИНАЦИИ С МИКРОСФОКУСИРОВАННЫМ УЛЬТРАЗВУКОМ В КОРРЕКЦИИ ВОЗРАСТНЫХ ИЗМЕНЕНИЙ НИЖНЕЙ ТРЕТИ ЛИЦА И ПОДЧЕЛЮСТНОЙ ОБЛАСТИ: ПРОСПЕКТИВНОЕ СРАВНИТЕЛЬНОЕ КОГОРТНОЕ ИССЛЕДОВАНИЕ EFFICACY OF CRYOLIPOLYSIS AND ITS COMBINATION WITH MICROFOCUSED ULTRASOUND IN THE CORRECTION OF AGE-RELATED CHANGES IN THE LOWER THIRD OF THE FACE AND SUBMANDIBULAR AREA: A PROSPECTIVE COMPARATIVE COHORT STUDY
Введение. Криолиполиз (КЛ) и микросфокусированный ультразвук с визуализацией (МСУЗ-В) являются эффективными и безопасными методиками для коррекции избыточно выраженной подкожно-жировой клетчатки (ПЖК) в подподбородочной (ППО) и поднижнечелюстной областях (ПНЧО), однако данные об эффективности их комбинированного применения в нижней трети лица ограничены.
Цель. Оценка эффективности монотерапии криолиполиза и комбинации с МСУЗ-В для улучшения качества кожи и сокращения объема ПЖК в ППО и ПНЧО.
Материалы и методы. В проспективное многоцентровое сравнительное нерандомизированное когортное клиническое исследование было включено 108 пациентов в возрасте от 35 до 65 лет. Исследуемая когорта была разделена на две контрольную и основную группы: в группе 1 КЛ (n=54) проводилась только процедура криолиполиза, в группе 2 КЛ+МСУЗ-В (n=54) – криолиполиз с последующим проведением МСУЗ-В через 3 месяца. Эффективность терапии оценивалась на основании клинических шкал (GAIS, выраженности ПЖК в ППО и ПНЧО), калиперометрии складок и суммарного индекса старения (СИС).
Результаты. В обеих группах наблюдалось статистически значимое уменьшение выраженности ПЖК в ППО и ПНЧО, а также снижение толщины кожной складки по данным калиперометрии. При этом комбинированный протокол был ассоциирован с более выраженными изменениями ряда показателей. Наиболее заметные различия между группами отмечались к 6-му месяцу наблюдения и сохранялись до 12 месяцев.
Заключение. Полученные результаты свидетельствуют о возможных преимуществах комбинированного применения криолиполиза и микросфокусированного ультразвука при коррекции избыточно выраженной ПЖК в ППО и ПНЧО и возрастных изменений нижней трети лица.
Background. Cryolipolysis (CL) and microfocused ultrasound with visualization (MFU-V) are effective and safe methods for the correction of excessive subcutaneous fat (SCF) in the submental and submandibular areas. However, data on the effectiveness of their combined use in the lower third of the face remain limited.
Purpose. To evaluate the efficacy of cryolipolysis as monotherapy and in combination with MFU-V for improving skin quality and reducing subcutaneous fat volume.
Materials and methods. A prospective multicenter comparative non-randomized cohort study included 108 patients aged 35–65 years. Participants were divided into two groups: group 1 (CL, n=54) received cryolipolysis only; group 2 (CL+MFU-V, n=54) received cryolipolysis followed by MFU-V after 3 months. Treatment efficacy was assessed using GAIS, SCF severity scales, caliper measurements, and the total aging index (TAI).
Results. Both groups demonstrated a statistically significant reduction in SCF severity and skinfold thickness. The combined protocol was associated with more pronounced improvements in several parameters. The most notable differences between the groups were observed at 6 months and persisted up to 12 months.
Conclusion. The combined use of cryolipolysis and MFU-V provides a more pronounced and sustained clinical effect compared to monotherapy.
