Together we try to create the journal’s reputation, to make it recognisable and readable. The end of the calendar year brings to mind the events of the past: inclusion of our journal in the list of Russian peer-reviewed scientific journals of the Higher Accreditation Commission (VAK); 4 issues of the journal with publications from 68 authors from 27 research, educational and medical organisations in Russia and abroad; 8 columns with presentation of clinical cases, economics and management of medicine and pharmacy, original articles, and reviews.

AUTOLIPOTRANSFER VOLUMIZATION OF INFRAORBITAL TISSUE VOLUME DEFICIT WITH A FREE FRAGMENT OF THE CHEEK FAT PAD (BISHAT) DURING LOWER EYELID PERIORBITOPLASTY

ABSTRACT

Background. The demand for plastic and aesthetic surgery is increasing year on year and so are the patient's expectations of the quality of the plastic surgeon's work. One of the most problematic areas on the face under the lower eyelids in ageing patients is the infraorbital region tissue deficit. To fill the local deficit of infraorbital tissue volume, free autolipotransfer volumization with a fragment of the cheek fat pad e (Bishat) is used in combination with periorbitoplasty of the lower eyelids. The most rejuvenating method for filling an expressed deficit of infraorbital tissue volume is lipofilling, which is not always sufficient to obtain the expected aesthetic result. The most effective predictable method for correcting the infraorbital tissue volume deficit is autolipotransfer volumization with a fragment of the fat pad of the cheek (Bishat) with its fixation with separate interrupted sutures to prevent its displacement.

Purpose. To evaluate and analyze the aesthetic volumizing and surgical results, as well as the frequency of possible complications when performing periorbitoplasty of the lower eyelids with free movement of a fragment of the cheek fat pad (Bishat) in the infraorbital region.

Materials and methods. The clinical study included 37 patients who underwent autolipotransfer corrective volumizing periorbitoplasty of the lower eyelids in combination with free movement of a single block of the cheek fat pad fragment (Bishat) to the infraorbital region, with fixation by separate sutures. The study involved female patients aged from 55 to 65 years. The main motivating reason for contacting an aesthetic surgeon in all patients was complaints of severe atrophy and volume deficit of infraorbital fatty tissue.

Results. On the 12th day after the surgery, 94.7% of the patients reported satisfaction with the aesthetic and cosmetic result. During the follow up examination of all operated patients two months later, 97.3% of the patients reported complete satisfaction with the cosmetic and aesthetic outcome of the surgery.

Conclusion. The results of aesthetic, cosmetic and surgical effectiveness of the performed lower eyelid periorbitoplasty in combination with autolipotransfer volumization by free movement of a fragment of the cheek fat pad (Bishat) to the infraorbital region were assessed. More than 95% of the operated patients reported a pronounced positive rejuvenating result after the surgical correction.

EPICANTHOPLASTY AS A KEY MANOEUVRE FOR CORRECTION OF ORIENTAL (ASIAN) EYELIDS IN COMBINATION WITH UPPER EYELID BLEPHAROPLASTY

ABSTRACT

Background. The number of patients with Asian eyelids (epicanthus) visiting aesthetic surgeons is increasing every year. According to Tyra Banks (an American supermodel and actress), a person's eyes convey their mood, emotions, state of inner harmony and soul. Patients set an aesthetic task for the surgeon, to remove the epicanthus and excess skin tissue above the upper eyelid to give the eyes a more European look, to make them look more open. Partial or incomplete removal of the epicanthus in combination with upper eyelid blepharoplasty will not achieve the desired aesthetic result. At first glance, it seeme to be a simple and technically uncomplicated operation, but at the same time it requires certain skills from the surgeon, has its own peculiarities of the postoperative course, contraindications, indications for its implementation, with frequent serious intraoperative and postoperative complications. The main task of the surgeon when performing Europeanization is the correction of the epicanthus, its removal, and medial release of the medial angle of the eye and anatomical restoration of the tissues in the place of the removed epicanthus. The combined aesthetically effective surgical method for correcting the epicanthus is upper eyelid blepharoplasty to achieve the most harmonious result of Asian eyelid Europeanization.

Purpose. To analyze and evaluate the early postoperative and long-turm aesthetic results, as well as the frequency of complications in the performed epicanthoplasty in combination with upper eyelid blepharoplasty.

Materials and methods. The study included 51female patients who applied to the plastic surgery clinic between 2023 and 2024. The age of the patients ranged from 24 to 29 years. All patients were of Asian origin. The patients showed aesthetic and ethnic dissatisfaction with the upper eyelids with the presence of pronounced epicanthus and the presence of excess upper eyelid skin. These complaints were the main motivating reason for performing epicanthoplasty and blepharoplasty of the upper eyelid. All patients underwent upper eyelid Europeanization, epicanthoplasty combined with upper eyelid blepharoplasty.

Results. In the immediate postoperative period, complications were noted in three patients (5%) on day10. At the six-month follow-up of all patients who underwent the surgery, two patients had a slightly unsatisfactory aesthetic result.

Conclusion. The cosmetic and aesthetic results of Europeanization surgery: epicanthoplasty in combination with upper eyelid blepharoplasty were analyzed. The majority of the operated patients (97%) had a positive aesthetic result after the surgery.

WORK PROCESS ORGANIZATION IN THE REGIONAL DISASTER MEDICINE & EMS CENTRE DURING RESPONSE STAGE OF AVIATION INCIDENT AT THE AIRFIELD

ABSTRACT

Background. According to the Interstate Aviation Committee, 21 aviation accidents were occurred in Russia in 2023, including 11 disasters in which 20 people died. During these incidents, 80% occurred due to human factors and were mainly related to a collision with a surface or obstacles in controlled flight.

Purpose of the studying was to demonstrate solutions of difficulties in work process out of the organizational form in interactions of medical forces and facilities of a civil airfield with the formations of the regional disaster medicine center during the response stage to an aviation accident with great number of casualties.

Material and methods. The analysis of the tactical and special exercises on evacuation, first aid, medical evacuation of casualties in aviation accident on the zone of Simferopol International Airport in August 30, 2024 was carried out with practical testing of the accepted "legend" the training exercise. According to the plan of the tactical special exercise, the Boeing-737 aircraft with 4 crew members and 27 passengers onboard made a hard landing and rolled out of the runway, during which the landing gear broke off, the aircraft tilted to the side and skidded to land.

Results. In the joint tactical and special exercises conducted on the territory of a civil airfield, the necessary actions were performed during the elimination of the medical and sanitary consequences of an aviation accident, with an emphasis on updating theoretical knowledge and improving practical skills among first-contact persons — airport employees (fire and rescue flight support services, passenger transportation services, emergency rescue teams of the airfield, airport healthcare center). As a result of the performed exercises, it was proposed to transmit the initial signal about the incident at the airfield to the control room of the regional disaster medicine service directly from the flight director, which will speed up the rapid response. Also, an analyzing of the practical implementation of the tasks of tactical and special exercises showed that the most difficulties were in is the registration of victims in the process of medical triage, which requires further work to optimize it. It is necessary to pay attention to the placement of groups of victims on the sorting area for the effective work of evacuation teams. For the effective work of the first contact persons, in particular, the passenger transportation service, it is necessary to periodically update knowledge and skills in the field of interaction with victims with an acute stress reaction.

Conclusion. The Crimean Disaster & EMS Centre, in order to increase preparedness for the elimination of medical and sanitary consequences at the airfield in an aviation accident, offers two elements of measures: 1) assistance in the development by the airport's operational services of practical skills in first aid, medical triage and preparation for medical evacuation; 2) ensuring the constant readiness of the regional disaster medicine service to provide emergency, including specialized medical care to victims of aviation accidents at the airport.

ABSTRACT

Background. The COVID-19 pandemic necessitated innovative approaches for effective treatment and rehabilitation of patients. This study explores the use of remote pulsed electromagnetic field (PEMF) therapy for virus elimination in the human oro- and naso-pharynx. For this purpose, a PEMP-based method was developed and implemented in the device “TOR”.

Purpose. To evaluate the safety and efficacy of the medical device when used for its intended purpose, as established by the manufacturer of “Non-invasive electromagnetic therapy device “TOR” according to BEMP.941523.001TU”.

Materials and Methods. This article reports on a randomized Phase II clinical trial evaluating this approach. In the trial, PEMF therapy was administered for five consecutive days, with a regime of 15-minute exposure with magnetic flux density of 1.5 µT, with peak electrode voltages 5-6.5 kV and pulse frequencies of 100-150 Hz from a distance of 5 metres from the patient to the TOR device. The therapy was combined with a standard anti-COVID treatment. A total of 236 patients were divided into three groups: control, experimental (activated “TOR” device) with standard therapy, and placebo (deactivated “TOR” device) with standard therapy.

Results. On the 5th day, 87% of patients receiving PEMF therapy had a negative PCR result, compared to 43% in the control group. By day 14th, this increased to 99% for the PEMF group, compared with 85% in the control group.

Conclusion. The combination of “TOR” non-invasive electromagnetic therapy device with standard COVID-19 treatment significantly accelerated patient recovery, with a high rate of PCR-negative results and no reported side effects. This trial is registered at ClinicalTrials.gov (NCT05220579). The “TOR” device was registered as a medical device in the State Register of the Federal Service for Surveillance in Healthcare of the Russian Federation (#RZN 2021/15459 dated September 23rd, 2021, register dossierNoRD36804/79643).

EXPERIENCE OF USING ALIMEMAZINE IN THE COMPLEX TREATMENT OF ROSACEA

ABSTRACT

Background. Rosacea is a chronic dermatosis primarily affecting the skin of the face, significantly negatively impacting patients’ quality of life.

Purpose. To assess the baseline levels of anxiety and depression in patients with erythematotelangiectatic and papulopustular subtypes of rosacea, as well as their combination, and to compare the effects of combined therapy on the psycho emotional state of patients.

Materials and Methods. An open prospective randomized study was conducted, including 30 patients diagnosed with erythematotelangiectatic (ETR) and papulopustular (PPR) subtypes of mild and moderate severity, as well as their combination. Depending on the choice of treatment method combinations, the patients were divided into two groups. In group No1: 15 patients received systemic therapy with minocycline and topical therapy with pimecrolimus cream. In group No2: 15 patients received additionally the drug alimemazine. The psycho emotional status was assessed using a questionnaire on autonomic disorders, a stress situation questionnaire, and the Hospital Anxiety and Depression Scale (HADS). The dermatological status was also evaluated before and after the treatment using the Diagnostic Symptom Scale Index and the Rosacea Diagnostic Assessment Scale according to V.P. Adaskiewicz, and dermatoscopy.

Results. Analysis of the questionnaire results showed high levels of stress, anxiety, and sleep disorders. All patients exhibited symptoms of anxiety and depression according to the Hospital Anxiety and Depression Scale (HADS). After completing the traditional treatment, a slight reduction in the mean values on the HADS scale was noted: the anxiety level decreased by 7%, and the depression level decreased by 14.6%. This indicates that the existing psycho emotional disorders did not undergo significant changes after traditional therapy. In the context of pharmacotherapy using alimemazine, there was a pronounced decrease in the mean values of the following criteria: the anxiety level decreased by 64.8%, and the depression level decreased by 65.7%. After the traditional treatment, an improvement in dermatological status was observed, confirmed by the results of the Diagnostic Symptom Scale Index (a decrease by 49%) and the Rosacea Diagnostic Assessment Scale (a decrease by 50%). Following the administration of alimemazine, more significant changes were observed: the Diagnostic Symptom Scale Index values decreased by 71.2%, and the Rosacea Diagnostic Assessment Scale by 69.9%, indicating a substantial regression of the most pronounced clinical symptoms of the dermatosis as a result of combined treatment. In both studied groups, an increase in the homogeneity of the microcirculatory bed was recorded after 4 weeks of therapy. However, in group No2 receiving alimemazine, the results persisted until the 12th week, while group No1 showed a decrease in microcirculatory homogeneity by the 12th week. The obtained data indicate a high clinical efficacy of the comprehensive approach to the treatment of rosacea.

Conclusion. Comprehensive assessment of autonomic and psycho emotional statuses included in the examination algorithm for patients with rosacea will successfully complement traditional therapy methods.

ABSTRACT

The study of products from oriental, mainly Chinese, traditional medicine has become particularly important in modern times. Tripterygium Wilfordii Hook F, which has been used for more than 2000 years to treat skin diseases, stands out among the remedies of ancient Chinese medicine. Intensive research is currently being carried out onto the mechanisms of action of various products of this plant. The data on the molecular and cellular mechanisms of the anti-inflammatory and immunomodulatory effects of these substances are especially important. The data obtained are of direct relevance to modern dermatological practice.

ABSTRACT

This article presents a comprehensive analysis of the risks and clinical outcomes of revision hip surgeries. The number of revision hip arthroplasties is increasing due to the demographic ageing of the population. The primary risk factors are age, the presence of comorbidities, and the condition of the bone tissue, which have a significant impact on the complication rate and long-term survival of the implants. Modern technologies and methods to improve the outcomes of revision arthroplasty are described, including the use of modular implant systems, 3D printing, and computer navigation. Special attention is given to the necessity of an individualized approach to patient treatment and further refinement of surgical strategies to reduce risks and improve patients' quality of life.

MIXED MELANOCYTIC NEVUS OF THE EXTERNAL AUDITORY CANAL

ABSTRACT

Melanocytic nevus is one of the most common benign skin neoplasms, consisting of cells representing progressive stages of migration and proliferation of melanocytes, while localization in the external auditory canal is extremely rare.

Patient N. (47 years old) was admitted with complaints of a neoplasm at the entrance to the right external auditory canal. The examination revealed that in the area of the entrance to the external auditory canal along its anterior wall there is a flat formation with a finely lumpy surface, protruding above the skin surface up to 2–3 mm, with clear edges on 2/3 of the circumference and a indistinct contour on 1/3, measuring 12x8 mm in size, pale pink in color with uneven areas of pigmentation. In the operating room under the microscope control, the mass was removed instrumentally within healthy tissues, free skin flaps were placed on the wound surface, covered with silicone strips, and the external auditory canal was tamponed with a hemostatic sponge. According to the histological examination, the specimen contained a mixed melanocytic nevus. After 14 days, the tampons and silicone strips were removed, the flaps were intact, and the wound surface was completely epithelialized. During 6 months of observation, there was no recurrence of the tumor or scar deformation at the entrance to the external auditory canal.

The presented clinical case of flat mixed melanocytic nevus of the external auditory canal is an extremely rare pathology for this localization, which required surgical treatment both for functional and aesthetic reasons, and for the purpose of differential diagnosis and exclusion of malignant transformation.

ABSTRACT

Progressive aging of the population, multiple comorbidities, irrational pharmacotherapy and insufficient knowledge of medical workers in the field of drug safety create high risks for elderly patients receiving medical care at the outpatient and hospital segments of health care. The development of adverse drug reactions in elderly patients is facilitated by the influence of the physiological characteristics of the aging body on the pharmacokinetics and pharmacodynamics of drugs, multimorbidity and polypharmacy, medical errors in prescribing and administering medicines, the characteristics of adverse interactions of simultaneously prescribed drugs and the complexity of treatment regimens. Close interaction between doctors of various specialties, clinical pharmacologists, patients and their families, increasing the level of knowledge and a patient-oriented approach will help minimize the risks of pharmacotherapy, improve the quality and length of life of elderly patients.

ABSTRACT

Background. As a rule, the proposed approaches of biofilm control, based on the existing experience of suppressing the vital activity of planktonic microorganisms, are not effective.

Purpose. To develop methodological approaches and technological solutions for the removal of biofilms and organomineral deposits in complex hydraulic systems of hemodialysis equipment, which can be achieved by searching for and approving optimal methodological and design parameters of the operation of electrochemical domestic devices.

Materials and methods. For the synthesis of metastable solutions of active substances with specified properties and their targeted use (disinfection and decalcification) the created unit AQUATRON-05-GDL-200 with a capacity of 200 l/h was used.

Results. It was shown that sediment particles dissolved completely within an hour in 0.5% citric acid solution at 78–84°C, while in 0.5 % citric acid solution with micro-supplement of supralimonoic acid complete dissolution within an hour occurs at 44–46°C.

Conclusion. The problem of guaranteed application of electrochemically modified citric acid solution not only for disinfection but also for decalcification and biofilm removal, which was not achieved by traditionally used citric acid solution, has been solved. The peculiarities of practical applications are related to the synthesis of metastable solutions with specified properties and their targeted use in biosystems at the place of use. The possibility of setting new tasks is associated with complex tests in complex hydraulic systems of various medical equipment.

ABSTRACT

Background. The paper presents a clinical case of a patient with eosinophilic granulomatosis with polyangiitis (EGPA) or Churg-Strauss syndrome with generalized lesion in the internal organs.

Purpose. To demonstrate the difficulties of diagnosing EGPA that a therapist may encounter.

Materials and methods. The Russian and international literature on EGPA and the clinical case of a patient with EGPA have been reviewed.

Results. The features of the clinical course of EGPA are described, the stages of diagnostic search for verification of the diagnosis in EGPA are given, the modern methods of treatment of patients with EGPA are considered. An example of a patient with EGPA is also given.

Conclusion. The described clinical case demonstrates the difficulties of diagnosing EGPA that a therapist may encounter.

ABSTRACT

This article is a systematic review on the topic of neurogenic sexual dysfunction. A systematic evaluation of the English-language literature up to 2024 was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The literature review focused on the following areas: sexual function regulation, neurogenic sexual dysfunction, and neurogenic erectile dysfunction.

The review demonstrated that in patients with central nervous system (CNS) disorders, neurological and urological diagnostics are recommended before initiating treatment for sexual dysfunction. The specific diagnostic methods for neurogenic sexual dysfunction were identified. The latest scientific data on sexual function regulation in both genders was summarized. The article listed the main therapeutic approaches to managing sexual dysfunction, including experimental techniques. The rehabilitation strategies for sexual function in patients with neurological pathology are discussed, with an emphasis on the key therapeutic methods.

Despite the fact that sexual dysfunction is a significant issue for patients with CNS disorders, high-quality evidence-based studies are exceedingly rare and mostly limited to data on the efficacy of phosphodiesterase type 5 inhibitors (PDE5Is) for the treatment of erectile dysfunction. Well-designed prospective studies for both genders are urgently needed to provide a more comprehensive understanding and effective management of sexual dysfunction in this patient population.

ABSTRACT

Background. Large prostate volume is one of the leading factors influencing postoperative functional outcomes after robot-assisted prostatectomy.

Purpose. To evaluate the dependence of functional results after robot-assisted prostatectomy on prostate gland (PG) volume.

Materials and methods. The results of a prospective study of the dependence of functional outcomes after robot-assisted prostatectomy on prostate volume are presented. From 2018 to 2023, one surgeon performed robot-assisted prostatectomy in 50 patients with Vph≥100 cm3 (group 1) at the National Medical and Surgical Center named after N.I. Pirogov. The comparative group also consisted of 50 people, but with Vpj<100 cm3 (group 2). For the reliability of the study, all patients underwent robot-assisted prostatectomy using the same technique — with bilateral preservation of the neurovascular e bundles of the pancreas. Before and after surgery, all patients underwent quality of life assessment using IPSS and ICEF-5 questionnaires.

Results. Before the surgical intervention, the patients filled the questionnaires of the IPSS scale (International Prostate Symptom Score). During the scoring of patients in group 1, the final score averaged 19±3 points, and in group 2, the same indicator was 14±3 points. The final result according to the IIEF-5 questionnaire in both groups was 20±2 points. After surgery, the International Conference on Incontinence Questionnaire-Short Form (ICIQ — SF) was used in both groups to determine the patients’ quality of life dependent on “urination disorder”. In group 1, the final result was 5±3 points, and in group 2, the same indicator was 4±1 points.

Conclusion. The large volume of the prostate gland has a significant impact not only on the complexity of robot-assisted prostatectomy, but also on the functional outcomes after it.

ABSTRACT

Background. Increased body mass index (BMI) is one of the main factors affecting the restoration of urine retention and erectile function after a robot-assisted prostatectomy (RARP).

Purpose. To determine the extent to which increased BMI affects functional results after robot-assisted prostatectomy.

Materials and Methods. From 2017 to 2023, a prospective study was conducted at the National Medical and Surgical Center named after N.I. Pirogov to determine the effect of increased BMI on the restoration of functional outcomes after a robot-assisted prostatectomy. The number of the patients included in the study was 100 people, divided into 2 groups equally. Group 1 had patients with increased BMI (≥30). Group 2 included patients with normal BMI. The quality of life of patients in both groups was determined using the IPSS (International Prostate Symptom Score) and IIEF-5 (International Index of Erectile Function) questionnaires. For the reliability of the study, all patients underwent RARP performed by one surgeon using the same technique with bilateral preservation of the neurovascular bundles of the pancreas.

Results. In order to determine the presence and severity of erectile dysfunction during the pre-treatment examination before the RARP, the IIEF-5 questionnaire was used. The final result in patients of group 1 averaged 17±3 points [14; 20], and in group 2 the same index was 22±2 points [20; 24]. After the expiration of the dynamic follow-up period, all patients re-completed the IIEF-5 questionnaire. Also during the study, the International Conference on Incontinence Questionnaire — Short Form (ICIQ — SF) was used to determine the patients’ quality of life dependent on “urination disorder”. In the postoperative period, the patients in group 1 had the final score of 10±3 [7; 13] points, and in group 2, the same indicator was 4±1 [3; 5] points.

Conclusion. Increased BMI is a predisposing factor for urinary incontinence and erectile dysfunction (ED) in the postoperative period after RARP.

ABSTRACT

The article presents 2 clinical observations of treatment of patients with stone encrustation and stone formation on the vesicular and pelvic coils of the internal ureteral stent placed after endoscopic stone removal. In both cases, for different reasons, there was a delay in planned hospitalization for stent removal.

ABSTRACT

Patient safety during surgical procedures has been a pressing issue for many years. In 2004, the World Health Organization (WHO) established the World Alliance for Patient Safety. This alliance has a campaign entitled “Safe Surgery Saves Lives” (2008). Healthcare-related injuries are iatrogenic and, unlike disease complications, can be prevented (WHO, 2019).

An integral part of surgery is the patient positioning on the operating table. The purpose of creating an optimal patient position during surgery is to ensure the best surgical and anesthetic access and, while at the same time preventing the development of potential complications associated with the patient's position. Any position of the patient on the operating table carries a certain degree of risk. This problem is aggravated by the fact that the patient is unable to inform about the complication due to unconsciousness and muscle relaxation.

This article examines the main types of patient positions on the operating table, describes the features of positioning, examines the complications and methods of preventing them.

ABSTRACT

The article presents the results of our own research and analysis of literature data regarding cases of duodenal obstruction as complication of chronic pancreatitis. The different approaches to the choice of surgical treatment methods for this type of surgical pathology are presented.

ABSTRACT

Background. Currently, diabetes mellitus (DM) is one of the main medical and social health problems. Most patients with type 2 diabetes do not have an opportunity to exercise regularly. At the same time, due to the multifactorial nature of the disease, patients with DM have to take up to 200 tablets per week, which can significantly reduce their adherence to treatment. The role of dietary therapy with glycemic-lowering foods is therefore even more important. In particular, these include products containing extracts of the herb Gynura, which have been proven to be effective in a number of laboratory studies.

Purpose. To determine the effectiveness of the product containing the herb Gynura in patients with type 2 DM.

Materials and methods. A clinical observation of the effectiveness of the product with the herb Gynura in patients with type 2 diabetes in addition to ongoing antidiabetic therapy for 21 days was carried out. A total of 23 patients participated, the average age of whom was 47.2∑5.5 years and disease duration was 6.1∑3.2 years.

Results. At the end of the consumption of dark chocolate “Gynura”, patients showed a significant decrease in body weight, improvement in glycemic indices, and a positive effect on liver function parameters. There were no negative effects on kidney and pancreatic function.

Conclusion. Daily intake of chocolate with the herb Gynura promotes weight loss and improves carbohydrate metabolism in patients with type 2 diabetes.

ABSTRACT

Background. This article is devoted to the analysis of a clinical case with a serious complication of chronic pancreatitis involving the wall of the superior mesenteric artery and the formation of a false aneurysm. Particular attention is paid to the possibilities of endovascular surgery in the treatment of this category of patients, as well as the causes and mechanisms of development of vascular complications.

Purpose. Research and analysis of the effectiveness of arterial stenting using a stent graft in identifying erosion of the visceral artery wall with fibrous capsule formation, and determining the possibility of its complex use with radiocontrast angiography to develop optimal treatment strategies for this pathology.

Materials and methods. The methodological basis of surgical interventions is determined by modern high-tech methods of cognition. This article uses structural-functional, logical and predictive methods of scientific knowledge.

Results. The analysis of the clinical case and data from instrumental diagnostic methods confirms that this method should be applied much more widely due to many advantages indicated in world practice.

Conclusion. The organization of highly specialized medical care implies the correct rational use of all modern treatment methods for patient’s treatment and early rehabilitation. The use of this stent graft implantation technique in highly specialized medical institutions currently requires careful analysis of diagnostic data in a favorable prognosis of endovascular interventions with discussion of each specific clinical situation by a multidisciplinary team of specialists, including vascular surgeons, doctors on X-ray endovascular diagnostic and treatment methods, cardiac surgeons and other specialists. The composition of the consultation will depend on the severity and area of the patient’s lesion, concomitant pathology, planned related surgeries and technical capabilities of the clinic.
