On behalf of the Editorial Board, I am delighted to present a new issue of our scientific medical journal dedicated to dermatovenerology, one of the most important and multifaceted fields of medicine. Dermatovenerology studies both skin diseases and the deep systemic pathologies that cause them. The skin is not just a protective covering, but also a complex indicator of the state of the body's internal organs and systems. This is why studying dermatovenerological diseases are so important for diagnosing, treating and preventing many other diseases.
In recent decades, dermatovenerology has experienced a period of rapid development, driven by significant progress in the pharmaceutical industry and the emergence of new, highly effective drugs. Today, dermatologists have a wide range of medicines to combat infectious and parasitic diseases, as well as acute and chronic non-infectious skin conditions. This significantly improves patients' psychological and physiological well-being and reduces the financial burden of treating skin diseases for both patients and society.
Modern methods of treating skin diseases often incorporate elements of cosmetic procedures and reconstructive surgery. This enables patients to not only overcome their condition, but also to restore their self-confidence and improve their appearance and quality of life. These areas of medicine are increasingly overlapping and complementing each other, with doctors and cosmetologists combining their efforts to achieve therapeutic and aesthetic goals.
Dermatovenerology currently remains a key discipline, combining knowledge of the skin as a reflection of overall health and integrating the achievements of pharmacology, cosmetology and surgery to take a comprehensive and effective approach to patients.

ABSTRACT
Background. Acne, traditionally considered as a disease, mainly of adolescence, has been increasingly reported in people over the age of 25 in the last decade. The features of the course of persistent forms of acne and acne with a late onset are resistance to therapy, persistent post-acne symptoms and a pronounced stress factor. Studying the process of neurogenic inflammation in this group of individuals is necessary for a prognostic understanding of the role and degree of stress as a factor preventing the stable remission of the disease.
Purpose. To search for correlations between the degree of neurogenic inflammation and stress levels.
Materials and methods. The study of neuropeptide content in blood serum was carried out by competitive inhibition ELISA method. An objective assessment of stress levels was carried out using the Holmes and Rage stress tolerance and social adaptation questionnairesand Spielberger-Hanin test of personality and situational anxiety.
Results. Neuropeptides characterizing the severity of neurogenic inflammation showed a statistically significant increase compared to the control.
Conclusions. The levels of neuropeptides in the acne group significantly exceeded those of the control group, confirming the association of neurogenic inflammation with the degree of stress.

ABSTRACT
Prostaglandins are an important class of biologically active substances involved in almost all functions of the organism. They play a special role in regulating inflammation, immune response, and cytokine synthesis. Among the known classes of prostaglandins, the pro- and anti- inflammatory functions of prostaglandin F2α, preparations of which are widely used in medicine, have been studied to a lesser extent. However, the involvement of PGF2α in inflammatory, fibrotic and degenerative processes makes it necessary to search for means of preventing or eliminating this undesirable effect. Such means may include products of traditional Chinese medicine plants, for example, Tripterygium Wilfordii Hook F. The obtained data are relevant to pharmacology and most areas of clinical medicine.

ABSTRACT
Background. Correction of age-related changes in the dorsal surface of the hands is a complex process that requires interdisciplinary interaction and combined treatment. This article presents modern data on the mechanisms of ageing, examples of diagnostics and assessment of the severity of ageing signs on the dorsal surface of the hands and an overview of modern correction methods.
Purpose. To acquaint medical specialists with modern methods of diagnostics and correction of age-related changes in the dorsal surface of the hands.
Material and methods. To select literature, the Internet search engines Google Shcolar, eLibrary, PubMed were used for 20 years by the search words: “hand aging-fillers-RF therapy-lasers”. The analysis included 58 articles, 53 of which belong to not Russian authors.
Results. Aesthetic correction of age-related changes in the dorsal surface of the hands requires a differentiated approach. The development of an individual plan for aesthetic correction should be based on the type of aging and morphological changes in the soft tissues of the dorsal surface of the hands.
Conclusion. The optimal results of aesthetic correction of the dorsal surface of the hands are achieved by combined methods, including hardware methods and minimally invasive methods of correction with fillers and skin boosters.

PSYCHOSOMATIC ASPECTS OF SKIN DISEASES AND THE INFLUENCE OF STRESS MANAGEMENT ON THEIR COURSE

ABSTRACT
This study examines the psychosomatic aspects of skin diseases and the impact of stress management on their course. The aim of the work is to analyze the relationship between psychological factors such as stress and the pathogenesis, clinical manifestations and effectiveness of treatment of dermatological diseases. The study used methods of systematic literature review, meta-analysis and statistical processing of clinical data of patients with various skin pathologies (n=1542). The results showed that the high stress levels significantly correlated with a more severe course of psoriasis (r=0.72; p<0.01), atopic dermatitis (r=0.68; p<0.01) and acne (r=0.65; p<0.01). The use of stress management techniques in complex therapy led to a more rapid regression of rashes and an improvement in the quality of life of patients (p<0.05). The data obtained indicate a significant role of psychosomatic factors in the development and course of skin diseases, as well as the prospects for including stress management methods in therapeutic protocols. The further research should be aimed at developing personalized stress management strategies and studying their long-term effectiveness.

ABSTRACT
Background. For 6 years (2018-2023), the features of the course of ophthalmorosacea were studied to develop an effective treatment method for this disease. The effectiveness of a new innovative therapy for ophthalmorosacea was evaluated in 182 people. The treatment consisted of the injection of 1% hyaluronic acid on special active points, followed by the application of 20% lactic acid in the form of peeling. This method of treatment allowed to eliminate the pathogenetic mechanisms of the ophthalmorosacea development. The method has been patented, and the Patent for invention No. 2810361 has been obtained.
Purpose. To develop a method for persistent stagnation of the inflammatory process in patients with ocular rosacea.
Materials and Methods. 182 patients were selected to analyze the effect of hyaluronic acid on the pathogenesis of ophthalmoacea. The results of the treatment method were observed from 2018 to 2023 inclusive.
Results. A method of anti-inflammatory, drainage, and regenerative therapy with 1% injectable hyaluronic acid for ophthalmorosacea was developed. An in-depth analysis and dynamic follow-up of patients were performed at home, 6 and 12 months after therapy, and the optimal amount and time interval between injection therapy were verified. The patient management strategy and tactics have been developed based on the Patent for invention No. 2810361. A highly effective and safe method of treating ocular rosacea with stable and prolonged stagnation of the process has been identified.
Conclusion. After carrying out 6 procedures for activating points of 1% hyaluronic acid, relief of ophthalmorosacea was observed in 98.2% of cases, all clinical signs of pathology were eliminated, and patients stopped using moisturizing eye drops.

ABSTRACT
Background. Patient N., 26 years old, complained of painful erosions on the mucous membrane of the mouth, lips, rashes on the scalp and skin of the trunk. On examination: pallor of the skin of the face, erosions on the mucous membrane of the lips and marginal gingiva, bleeding when touched; marginal gingiva with signs of desquamation; erosion with fragments of epithelium in the soft palate; positive Nikolsky’s sign. The patient was examined, confirming the pemphigus vulgaris (PV) . The treatment led to an improvement in the patient's condition. The patient was discharged under the supervision of a dermatovenerologist at his place of residence with recommendations on reducing the dose of prednisone and dietary intake. The presented clinical case showed difficulties in the diagnosis and treatment of PV and the prevention of exacerbations. The authors of the article studied all the main processes associated with the prerequisites and occurrence of pemphigus vulgaris in men. The methods of diagnosis and differential diagnosis of PV were highlighted. The identified pemphigus vulgaris in the clinical case was examined from the point of view of diagnosis, treatment and prevention of the disease.
Purpose. To review a complex clinical case and familiarize medical professionals with such a rare and complex disease as pemphigus vulgaris. Materials and methods. The 26-year-old patient, chosen for the case, was initially misdiagnosed and improperly treated. As a result of the work of highly qualified dentists and dermatovenerologists, the diagnosis was correctly made and adequate treatment was prescribed, leding to improvement in the patient's condition.
Results. The patient's condition steadily improved as a result of adequate treatment.
Conclusion. A correct and timely diagnosis and conducted treatment, including supportive therapy, are the key to preserving the life and working capacity of patients.

ABSTRACT
Background. Patient A., 40 years old, complained of severe pain, burning of the face, a feeling of “fire” on the face after any emotions and during sleep, which did not go away on their own (the patient had to apply a wet towel, previously cooled in the freezer to fall asleep), sleep disorders, lack of appetite, redness in the eye area, cheeks, swelling of the face, crusts on the face in the medial areas of the cheeks, lower eyelids of both eyes, large-scale peeling of the skin in the area of the crusts. The patient was concerned about the increased photosensitivity of the eyes, constantly squinting her eyes at bright light (including sunlight), lacrimation, white and sometimes yellow discharge from the eyes, a feeling of “sand in the eyes”, decreased vision. On the examination: A V-shaped lesion of the cheek skin of a symmetrical nature, the inflammatory process affects the skin of the lower eyelids, erythema multiforme, crusts on the affected area, traces of excoriation, eversion of the lower eyelids due to pronounced swelling of the tissues. The V-shaped area was hot to the touch, xerosis, hyperkeratosis were visualized, a sharply positive leaflet symptom (the area is painful on palpation), a positive palpation test. The therapy was performed in accordance with the Patent for invention No. 2810361. 1% hyaluronic acid was injected into the active projections, followed by the application of 20% lactic acid to the activation zones. During 36 months of follow-up, the patient had no complaints of an inflammatory process in the V-shaped lesion area of the cheeks and eyelids. The presented clinical case of ophthalmorosacea is non-trivial, shows the variability of pathology, the psychoemotional background was of a stalemate with suicidal thoughts.
Purpose. To analyze an unusual clinical case complicated by an incorrect strategy of previous therapy, to identify an effective method of treating a patient with ophthalmorosacea, and to eliminate complications after incorrect cosmetological care.
Materials and Methods. To understand the cause-effect relationship and to analyse in the case of a 40-year-old patient A. who had negative dynamics from the prescribed therapy. The patient provided all the data on the intake and use of medications, course intervals, cosmetic procedures and cosmetics. The injection therapy was performed according to the Patent for invention No. 2810361. 1% hyaluronic acid was injected, followed by application of 20% lactic acid.
Results. The result was observed after each procedure: 6, 12, 24 and 36 months. The inflammatory process was stopped after 4 procedures (6 were performed as prescribed to obtain a fixed result), after 12, 24 and 36 months the condition of the patient's eyes, eyelids and skin were intact, the patient did not notice any relapses.
Conclusion. After the injection therapy with 1% hyaluronic acid and the use of 20% lactic acid, all signs of inflammation of the eyes, eyelids and facial skin were relieved.

ABSTRACT
Background. The study is dedicated to evaluating the effectiveness of aesthetic correction of age-related changes in the soft tissues of the periorbital area using intradermal injections of a polynucleotide-based skin booster.
Purpose. To develop an algorithm for the use of skin boosters based on PolyPhil polynucleotides (Croma, Austria) for the correction of age-related changes in the soft tissues of the periorbital region and to evaluate the aesthetic result of the correction using skin profilometry. Materials and methods. The proposed method of aesthetic correction was tested on 23 patients. The group included 18 women and 5 men aged from 18 to 55 years with a skin phototype according to the Fitzpatrick scale types 1-5. All patients underwent a course of intradermal administration of the PoliPhil skin booster (Croma, Austria) using a blunt 22 G cannula 7 cm long (SoftFill, Switzerland), 0.01-0.02 ml microbolus technique, total volume of 2 ml, distributed on both sides of the periorbital areas of the face.
Results. The effectiveness of the treatment was assessed 2 months after the end of the course of aesthetic correction as part of the follow-up program. On average, the depth of the nasolacrimal groove decreased from 0.1mm to 2 mm.
Conclusion. The minimally invasive method of aesthetic correction of age-related changes in the soft tissues of the periorbital region using intradermal injection of the polynucleotide-based skin booster is an effective and safe procedure that can be successfully carried out by cosmetologists and plastic surgeons.

ABSTRACT
Background. A fire in the operating theatre is a relatively rare event. However, in the event of a fire, the effects can be catastrophic if a patient is burned or injured, with serious legal and economic consequences for both the surgical team and the institution as a whole.
The statistics of fires in medical institutions in our country in recent years are quite alarming. According to the Ministry of Emergency Situations, there were 110 fires in healthcare institutions in 2019.
Main Content. Fire safety rules in the operating theatre should be strictly adhered to by all members of the surgical team. Operating room fires can be catastrophic, resulting in patient injury or even death. It is important that all operating theatre staff are aware of the behaviour rules in the event of a fire in the operating theatre. For this reason, fire safety training should be conducted regularly and is an important element of fire prevention.
Conclusion. This article discusses the causes of fires in operating theatres, sources of ignition and combustible materials. Recommendations for preventing and extinguishing fires for all members of the operating theatre team are given.

ABSTRACT
Background. The development of local treatment of long-term chronic trophic ulcers in severe obliterating atherosclerosis is a relevant problem of modern surgery.
Purpose. To study the effectiveness of nafion-based composite bandages in the treatment of patients with critical lower limb ischemia and long- term non-healing trophic ulcers.
Materials and methods. The controlled, non-randomized study involved 30 people with a trophic defect of the lower leg or foot who underwent arterial reconstructions at the Department of Vascular Surgery of the Republican Clinical Hospital of the Ministry of Health of the Kabardino- Balkarian Republic. The first group (n=13) consisted of patients who got bandages with nafion–based composite material once every 2 days in the postoperative period, the second group (n=17) consisted of patients who were bandaged with Levomekol ointment or iodopyron. The duration of the inpatient and outpatient stage of bandaging was 30 days. The study assessed the healing rate of trophic ulcers, as well as the microbial landscape on its surface. The SPSS Statistics 17.0 software was used for statistical data processing when comparing study groups.
Results. In the first group of the study, trophic healing by day 30 was observed in 25.4% of cases, while in the second group of the study — in 17.2%. (p < 0.05). The rate of healing of trophic ulcers in the first 6 days of the study was comparable, but subsequently there was a significant increase in the rate of regeneration in the experimental group (p < 0.05). At the same time, from day 9 in the first group of the study, there was a significant growth of mature granulation tissues, accompanied by a decrease in fibrinous overlays and necrosis. On the 10th day, there was a decrease in pathogenic flora in the main group. In the control group, the level and spectrum of contamination remained the same, only 11.4% of the subjects showed a decrease in microbial associations of E. coli to 102 CFU/tamp. It should be noted that this trend persisted in the study groups up to 21 days.
Conclusion. The use of composite bandages with nafion in the treatment of trophic ulcers after arterial reconstructions performed for critical lower limb ischemia accelerates the healing process.

ABSTRACT
Background. Due to the limited amount of local skin cover and the specific functional load on this area, closure of soft tissue defects of the foot is a significant surgical problem. However, the use of the sural flap can achieve significant success in solving this problem.
Purpose. To review in the framework of a clinical case the use of a sural flap in closing an extensive soft tissue defect in the foot region to restore the integrity of the traumatized soft tissues and ensure good functional and aesthetic rehabilitation of the patient.
Materials and methods. A 23-year-old patient brought to the 57th Department of Plastic and Reconstructive Surgery of the Central Military Clinical Hospital named after A.A. Vishnevsk during the evacuation phase. The patient was traumatized in the combat zone by stepping on the anti-personnel mine “Lepestok”.
Results. In the postoperative period the flap on the first two ties showed its arterial and venous integrity during 4 postoperative days. As a result of the performed measures the risk of the flap marginal necrosis and epidermolysis development was excluded.
Conclusions. The use of the sural flap for the closure of soft tissue defects of the foot demonstrates high efficiency and is the optimal choice in certain clinical situations.

ABSTRACT
Background. One of the problems of the lower eyelids in elderly patients in ophthalmic and plastic surgery is ectropion, eversion of the lower eyelid of atonic etiology. The occurrence of atonic lower eyelid ectropion is connected with impaired innervation of the zygomatic branch of the facial nerve or is formed due to involutional changes in the tarsal plate of the lower eyelid during its degenerative changes. In addition to the aesthetic and cosmetic defect of the lower eyelid, there is a risk of developing keratitis on the part of the cornea of the eyeball. The most effective and radical method of treating ectropion of atrophic origin is surgical. Various methods of surgical treatment of atonic lower eyelid ectropion have been published in a scientific literature review. An optimized, effective method of surgical correction of lower eyelid ectropion is shortening the lower eyelid lamella and suspending it to the lateral wall of the orbit, which creates a stable and long-term result. Observations in the long-term postoperative period with positive clinical results have been analyzed, but they require further study and possible improvement.
Purpose. To analyze and provide the aesthetic and therapeutic effectiveness of surgical correction of atonic lower eyelid eversion, by shortening the lamella of the tarsal plate of the lower eyelid and suspension (suturing) to the lateral wall of the orbit.
Materials and methods. The clinical study involved 15 patients who underwent a corrective operation between 2023 and 2024, shortening the overstretched tarsal plate of the lower eyelid and fixing it with separate sutures to the lateral surface of the orbit (Lateral Tarsal Strip-LTS). The study involved female and male patients, their age ranged from 70 to 75 years. The main motivation for contacting an aesthetic surgeon for all patients was complaints of lower eyelid sagging, incomplete closure of the palpebral fissure, lacrimation, conjunctivitis, and keratitis.
Results. After the corrective operation, on the 3rd day, the most of the patients (94.7%) noted satisfaction with the aesthetic and cosmetic result. During a follow-up examination of all operated patients after three months, 98.3% of patients noted complete satisfaction with the functional and aesthetic result of the corrective operation performed (Lateral Tarsal Strip-LTS).
Conclusion. The functional and aesthetic efficiency of the corrective surgery aimed at shortening the tarsal plate and fixing it with separate sutures to the lateral surface of the orbit (Lateral Tarsal Strip-LTS). More than 96% of operated patients noted a pronounced positive functional and aesthetic result after surgical correction of atonic lower eyelid ectropion.

ABSTRACT
Background. According to modern scientific research, patients with cervical spine dorsopathies have a significant decrease in mobility indices from 39.3% to 67.6% of the standard age indicators and the development of pain syndrome. Neuromuscular activation methods can significantly improve the above-mentioned parameters of the neck muscles, which are additionally involved in the movements of the shoulder girdle, which leads to correction of the pathological motor stereotype in patients with cervical dorsopathies.
Materials and methods. The study involved 64 people, including 28 women and 36 men, aged from 27 to 60 years. All patients were diagnosed with dorsopathy of the cervical spine. The patients underwent all rehabilitation measures on an outpatient basis for 4 weeks.
Results. The use of neuromuscular activation methods in addition to physical therapy aimed at relaxing the muscles of the cervical spine, medical massage and electrotherapy with diadynamic currents leads to a significant improvement in active mobility and a decrease in the severity of pain syndrome in patients with chronic manifestations of cervical dorsopathy.

Background. In the modern world, cancer remains one of the main causes of disability and mortality. Primary malignant neoplasms of the female reproductive system are an urgent medical, social, socio-economic problem associated with high rites of morbidity, prevalence, disability and mortality. The effectiveness of rehabilitation of disabled people of this category and the improvement of their quality of life depend on the compliance of the used technologies with the degree of severity of persistent clinical and functional disorders of the body and the dynamics of their changes, leading to different severity of disabilities. In this regard, the study of clinical, functional and expert-rehabilitation characteristics of disability is of particular relevance.
Purpose. To assess the limitation of the main categories of life activity and the degree of their severity on the basis of clinical and functional disorders of the body functions of the disabled due to metachronous and synchronous malignant neoplasms of independent (primary) multiple localisations of the female reproductive system.
Materials and methods. The study was formed in xlsx format in the R** environment. For continuous normally distributed variables, the mean value and standard deviation were calculated. The t-test was used when comparing two groups. For continuous abnormally distributed variables, the mean value (M) and standard deviation (SD) were calculated, and the p value of the Student's t-test (p_t) was determined when comparing two groups. For continuous non-normally distributed variables, the median (Me), the 1st (Q1) and 3rd (Q3) quartiles were computed, and the Mann-Whitney U-test and its p value (p_mu) were used when comparing groups. For each pair of such variables, Spearman's correlation coefficient and its p value (r (p)) were calculated. The Chi-square test of the corresponding contingency table and its p value p(χ2) were used to identify the relationships between the categorical variables.. If for some cells in the table the expected frequency was <5, then the Yeats correction for continuity (https: www.r.project.org)) was applied.
Results. The predominance of people over the working age (73.0%) was found, with middle-aged disabled people (19.8%) on the second place, as a result of the analysis of the limitation characteristics of life activity and persistent impairments of the body functions of disabled people due to metachronous and synchronous malignant neoplasms of independent (primary) multiple localizations of the female reproductive system. 42.0% of people had higher education, 39.0% had secondary education, 14.0% had general secondary education and 5.3% had general basic education. The average height of the disabled was 163 cm, the weight was 75 kg and the body mass index was 28.0 kg/m2. Disabled people of group II prevailed (53.5%). The largest proportion were disabled people with malignant neoplasms of the 2nd degree of malignancy (46.5%). In terms of malignant neoplasms, the metachronous type of cancer prevailed (60.1%). In terms of malignancy, no statistically significant differences were found between sychronous and metachronous cancer. The first stage of the disease was most often observed when cancer was localised in the ovaries (84.6%), uterine body and fallopian tube. The second stage was noted in the mammary gland (85.5%), the third stage was in the mammary gland (57.7%) and ovaries and the fourth stage was in the cervix (66.7%) and ovaries. It has been established that the higher the stage of the disease, the more severe the disability group. The combination of localisation of malignant neoplasms in the mammary gland and uterine body was most often found (28.9%). Blood and immune system dysfunctions were frequently found, mostly to a pronounced degree (52.0%). Of all the disorders affecting the body functions of the disabled, only disorders of the cardiovascular system had a statistically significant correlation with age r(p)=0.24 (<0.001). Blood and immune system disorders had a direct strong correlation with work activity (r=0.88). The ability to work was found to have the same correlation with total impairments (r=0.83). The highest proportion (72.7%) was self-care restrictions of the first degree.
Conclusion. The study of the clinical, functional and expert rehabilitation features that lead to persistent impairments of body functions and pronounced limitations in disabled people’s life activities due to metachronous and synchronous malignant neoplasms with independent (primary) multiple localizations in the female reproductive system, and the establishment of a correlation between these disorders and limitations in activities, contributes to the development of differentiated approaches to the medical and social examination and rehabilitation of the disabled.

MODERN TECHNOLOGIES IN REHABILITATION OF PATIENTS AFTER AMPUTATIONS OF BOTH LOWER LIMBS DUE TO COMBAT INJURY: THE FIRST EXPERIENCE OF USING AN ANTIGRAVITY TREADMILL

ABSTRACT
Background. The growth of the population with lower limb amputation defects is of high social and economic importance and determines the importance of introducing innovative technologies to improve the effectiveness of rehabilitation of this category of patients.
Purpose. To evaluate the effectiveness and safety of medical rehabilitation using an antigravity treadmill in patients at the stage of primary prosthetics after lower limb amputations due to combat injuries.
Materials and Methods. The case series (n=5) analysis was conducted on the rehabilitation of patients following bilateral below-knee amputations due to combat trauma. The clinical and functional status of patients was examined along with the presence of factors, potentially limiting rehabilitation. The safety and effectiveness of new medical rehabilitation program, which included 10 sessions on an antigravity treadmill over a two-week period, were assessed.
Results. All patients completed a course of 10 training sessions on an antigravity treadmill over two weeks and achieved their individual rehabilitation goals. All patients improved the key walking patterns and demonstrated more than a 50% increase in walking speed. Balance and coordination functions enhanced by 22-42% and 14-33%, respectively. The Hauser Walk Index increased by 2 points, while the Rivermead Mobility Index improved by 2 to 5 points. No adverse events were reported.
Conclusion. The use of antigravity treadmill in rehabilitation of patients with bilateral below-knee amputations is characterized by favorable safety profile. This type of training facilitates compensation for major limiting factors and effectively supports achieving rehabilitation goals related to walking function.

ABSTRACT
Background. The active development of biotechnologies opens new opportunities for overcoming male infertility. This literature review presents promising techniques that may soon be implemented in medical practice in patients with non-obstructive azoospermia.
Materials and methods. A search, analysis and systematization of relevant medical publications in PubMed and eLibrary databases was carried out using the keywords «мужское бесплодие» (“maleinfertility”), «лечение» (“treatment”), «новые технологии» (“up-to-datetechnologies»”). 27 publications were selected and included in this review.
Results and discussion. To overcome infertility in men with spermatogenesis arrest, the possibility of using spermatocytes of the 2nd order and round spermatids for intracytoplasmic injection is studied. The possibility of inducing the development of spermatozoa from a somatic pluripotent stem cell in vitro is also investigated. In addition, an organotypic culture (a fragment of testicular tissue) can be used to carry out spermatogenesis in vitro, for which 2D-and 3D-systems are used.
Conclusion. New technologies for overcoming male infertility give a chance of fatherhood to patients with non-obstructive azoospermia who do not have mature spermatozoa. Their development and future implementation in everyday practice remains the subject of research and active discussions.

ABSTRACT
Background. Approximately 15% of men are diagnosed with varicocele and from 2% to 10% of them have pain in the scrotum. Varicocelectomy is still the preferred option for scrotal pain management. However, the issue of assessing the prognostic factors for the success of surgical treatment, selection of patients for surgical correction of varicocele, as well as the choice of the surgical treatment technique itself remains the subject of discussion.
Materials and methods. The search of relevant publications was carried out in PubMed and e-Library databases using the keywords “varicocele”, “pain”, “scrotal pain”, “surgical treatment”. After excluding conference abstracts, dissertations and their abstracts, 27 sources were selected for inclusion in the literature review.
Results and discussion. Varicocelectomy in carefully selected patients with clinically palpable varicocele eliminates pain symptoms in most patients. Microsurgical methods of varicocelectomy remain the most preferred in the treatment of patients with scrotal pain associated with varicocele. Limited data suggest that the degree of varicocele, the nature and duration of pain, body mass index, previous conservative treatment, and the type of surgical technique used are predictors of varicocelectomy outcome.
Conclusion. Currently, the pathogenesis of scrotal pain in patients with varicocele remains poorly understood. Standardisation of the selection criteria for patients with scrotal pain caused by varicocele, and assessment of the predictive factors for the success of a particular method of surgical correction of varicocele are necessary.

ABSTRACT
Background. Varicocele is a common cause of male infertility, and it may affect the outcomes of ART (assisted reproductive technologies). Surgical treatment of varicocele may increase the likelihood of success of ART, but to date there are limited studies evaluating the results of intrauterine insemination (IUI), in vitro fertilisation (IVF) and intracytoplasmic sperm injection (ICSI) after varicocelectomy.
Materials and methods. We searched, analysed and systematised publications in PubMed and e-Library databases using the keywords «мужское бесплодие», «фрагментация ДНК сперматозоидов», «варикоцеле», «ВМИ», «ЭКО», «ИКСИ», “male infertility”, “sperm DNA damage”, “varicocele”, “IUI”, “IVF”, “ICSI”. 27 publications were included in the review after exclusion of conference abstracts, theses, dissertations and their abstracts.
Results. Only 3 controlled studies comparing pregnancy and live birth rates from intrauterine insemination after varicocelectomy and in the absence of surgical treatment were found. Only one of these studies found a statistically significant increase in these rates. The results of most of the studies evaluating the effect of varicocelectomy on the effectiveness of IVF/ICSI were in favor of surgical treatment before the ART procedure. However, the studies were mostly retrospective, which did not allow drawing an unambiguous conclusion.
Conclusion. Despite the insufficient number of randomized controlled trials confirming the effect of varicocelectomy on pregnancy and live birth rates during IUI, IVF and ICSI, it is advisable to perform this intervention before using ART.

ABSTRACT
Background. Drug therapy of benign prostatic hyperplasia (BPH) in the form of a combination of alpha-1 blockers and 5-alpha-reductase inhibitors is the standard of conservative treatment of this disease. However, it is known that every seventh patient may experience clinical progression of the disease during 4 years on the background of drug treatment, which entails the need for surgical treatment of BPH in 5% of men.
Main Content. The study of additional factors of BPH pathogenesis is an important problem. To date, it is known that existing microbial communities may be a risk factor for the development, proliferation and progression of benign prostatic hyperplasia. This article presents an analysis of the current literature on the association of changes in the composition of the gut and urine microbiome with BPH, as well as the effect of the prostate microbiome on lower urinary tract symptoms (LUTS).
Conclusion. Changes in the balance of the prostate microbiome in patients with BPH can provoke the occurrence of LUTS and exacerbate the clinical picture of the disease due to the direct effects of microbes on prostate tissue, bladder and urethral mucosa, activation of local immunity, and modulation of androgen metabolism.
The state of the microbiological landscape of the prostate is normal, with its inflammation and hyperplasia still poorly understood today. Therefore, a deeper understanding of this problem will help shed light on understanding new factors of prostate physiology and pathology, as well as improve treatment outcomes for patients with BPH.

TYPE 1 TYMPANOPLASTY FOR CHRONIC TUBOTYMPANAL SUPPURATIVE OTITIS MEDIA

ABSTRACT
Background. Chronic suppurative otitis media (CSOM) is a socially significant disease, the need for surgical treatment of which is beyond doubt. The most common surgery for CSOM is tympanoplasty.
Purpose. To conduct a comparative analysis of the results of type 1 tympanoplasty using three types of autografts: auricular cartilage, thin plates of auricular cartilage, and temporal fascia.
Materials and methods. The study involved 60 patients with tubotympanic suppurative otitis media, who were divided into 3 groups of 20 people: group 1 underwent type 1 tympanoplasty using thin plates of auricular cartilage, group 2 underwent type 1 tympanoplasty using temporal autofascia, and group 3 underwent type 1 tympanoplasty with a graft of auricular cartilage with perichondrium. The patients underwent preoperative examination: collection of complaints and anamnesis, examination of ENT organs using microscopic and endoscopic equipment, clinical and laboratory examination, multispiral computed tomography (MSCT) of the temporal bones, hearing assessment using pure tone threshold audiometry at frequencies of 500-1000-2000-4000 Hz. The postoperative follow-up examinations were performed after 1, 3, 6 and 12 months.
Results. In group 1 (13 men, 7 women), the average age was 36.1±7.7 years, the number of perforations no larger than 1 quadrant of the eardrum was 3 (15%), more than 1 quadrant but less than three were 12 (60%), total perforation was 5 (25%), the average air-bone gap (ABG) was 33.5±5.6 dB. In group 2 (14 men, 6 women), the average age was 39.2±6.8 years, the number of perforations smaller than 1 quadrant of the eardrum was 4 (20%), more than 1 quadrant but less than three was 13 (65%), total perforation was 3 (15%), the average ABG was 30.5±4.2 dB. In group 3 (15 men, 5 women), the average age was 37.2±7.3 years, the number of perforations smaller than 1 quadrant of the eardrum was 2 (10%), more than 1 quadrant but less than three was 12 (65%), total perforation was 5 (25%), the average ABG was 29.4±6.5 dB. One year after the surgery in group 1 the neotympanic membrane was competent in 95%, the average ABG was 11.1±4.9 dB. In group 2 the neotympanic membrane was competent in 85% (17 patients) with three residual perforations in patients with total perforations, ABG was 12.3±5.2 dB. In group 3, there was 1 case of failure with the formation of a slit perforation with a total success rate of the operation of 95%, ABG was 14.4±8.9 dB. In all groups there was a significant improvement in hearing compared to the preoperative results (p<0.05), while there was no significant difference in the degree of hearing improvement between the groups. However, in group 2, in contrast to groups 1 and 3, there was a retraction pocket formation in 2 cases (10%) and , in 1 case (5%), lateralization of the neotympanic membrane with the formation an atresia of the meatotympanic angle.
Conclusion. The results of the study showed no statistically significant difference in the number of residual perforations of the tympanic membrane and the results of the hearing test after surgery according to the tonal threshold audiometry at frequencies of 500-1000-2000-4000 Hz. At the same time, in group 2, in contrast to groups 1 and 3, there were cases of retraction pockets formation and lateralization of the neotympanic membrane.

EXPERIENCE OF USING THE INNOVATIVE MEDICINAL HEPATOPROTECTOR GEPAFOR® WITH A PROBIOTIC COMPONENT IN THE TREATMENT OF ALCOHOLIC LIVER DISEASE

ABSTRACT
Background. The positive effect of the combined use of hepatoprotectors and probiotics in acute viral hepatitis was noted in the work of L.G. Pyatov and V.A. Martynov. However, until recently, there was no drug on the pharmaceutical market that contained hepatoprotector and probiotic in a single dosage form. Hepatoprotectors are traditionally used to restore liver cells, more than half of which are herbal preparations. The interest in them is caused by the variety of mechanisms of protective action, a wide range of applications, easy and rather complete assimilation in the body, high safety and good tolerability.
Purpose. To evaluate the effectiveness of Gepafor® in patients with alcoholic liver disease (ALD) developed against the background of the use of ethanol-containing beverages.
Materials and methods. The examined group included 48 patients with mild to moderate ALD and impaired quantitative and qualitative composition of the microflora. The patients of the main group (24 people) received Gepafor® (2 capsules 3 times a day for 14 days) for the treatment of alcoholic liver disease. The patients of the comparison group (24 people) received the drug Silimar® for the treatment of ALD (2 tablets 3 times a day for 14 days). The condition of the liver and the manifestation of dysbiotic disorders were assessed by clinical and laboratory parameters before the appointment of hepatoprotectors and on the 14th day of their use.
Results. The use of Gepafor® for 2 weeks allowed to provide a decrease in the average level of transaminases in patients of the main group: alanine aminotransferase from 53.8±8.4 to 24.7±8.5 U/l; aspartate aminotransferase from 57.25±9.0 to 26.1±6.6 U/l (p< 0.05). This correlated with the disappearance of the clinical manifestations of ALD and an improvement in the composition of the intestinal microflora. The latter was evident in an increase in the number of people exhibiting minimal first-degree dysbiosis (from 33.7 to 58.5) and a decrease in those with severe dysbiosis -of the third degree (from 25.0 to 5.3).
Conclusion. The results of the observations show that therapy with the combined drug Gepafor® is more effective than therapy with the monodrug Silimar® on the background of basic therapy, since Gepafor® has a positive effect on the main clinical indicators, most biochemical indicators and indicators of the intestinal microflora, forming a tendency towards normalization of the intestinal microbiocenosis.
